《保险研究》20250905-《按病种付费医保支付方式改革的双刃剑效应分析——基于多时点DID的实证分析》（朱晓佳、于淼）

［摘   要］本文利用2011~2022年四川省城市级面板数据，将按病种付费医保支付方式改革作为政策冲击变量，评估其对医院运营的影响。基于多时点双重差分模型的分析表明，按病种付费改革对医院运营具有双刃剑效应：一方面降低医院次均住院费用、缩短平均住院日、提高病床周转次数；另一方面使医院产生了转移门诊、分解住院、增加处置强度等策略性行为，具体表现为改革城市医院次均门诊检查治疗费用、住院率和手术率显著提高。上述效应随着改革推进不断强化，且受到老龄化率和医疗资源水平的调节。研究表明，医院可能通过策略性行为来应对改革，为提升改革的有效性，需进一步完善付费政策和监管制度。

The Double-edged Sword Impacts of DRG/DIP Payment Reform：A Multi-Time-Point DID Empirical Assessment

Abstract：Utilizing city-level panel data (2011-2022) from Sichuan Province，this study evaluates the effects of the DRG/DIP payment reform，as a policy shock variable，on hospital operations.Using a quasi-experimental multi-time-point DID model，the analysis reveals the double-edged sword impact of the reform.On one hand，DRG/DIP implementation significantly lowers the average inpatient cost and length of stay，while improving bed turnover rates.On the other hand，it triggers strategic behaviors of hospitals such as shifting costs to outpatient services，decomposing hospitalization，and increasing treatment intensity.Specifically，post-reform increases are observed in outpatient examination/test costs，hospitalization rates，and surgical rates in pilot cities.These effects intensify over time and are moderated by population aging and regional medical resource availability.Our analysis also shows that hospitals may response to the reform through some strategic behaviors.To enhance the effectiveness of the reform，we should further refine the payment policy and regulatory frameworks.
